Accommodations Appeals Form

Students who feel that a provided accommodation or the denial of an accommodation is not appropriate, reasonable, effective, or is discriminatory, may submit an appeal to the Accommodations Appeals Committee, which includes the Dean of Students or designee, the Assistant Dean of Academic Services, and the General Counsel. The appeals process provides an opportunity for the student to present additional information or documentation. The decision of the Accommodations Appeals Committee will be final.

Clearly state the reason for your appeal. Provide a brief overview of the decision you are appealing and explain why you believe it should be reconsidered or modified.

Outline the original accommodation request you made, including any supporting documentation you provided at the time - you may include additional documentation as an email attachment with this form.

Provide detailed information/evidence to support your appeal, including relevant laws or regulations, medical documentation, or any other pertinent information.
Include any new information and/or documentation if relevant to your appeal.
